What is Care Plus?

Care Plus is a new program at UCSF with the goal of better supporting people and their family members who are facing serious and chronic illness. Eligible cancer patients and primary care patients will be contacted by our team.

What services are offered through Care Plus?

Some of the services offered through Care Plus include:

  • Advance Care Planning Resources
  • Social Work Consultations
  • Emotional Support for Patients
  • Caregiver Resources
  • Symptom Management

How do I contact Care Plus?

We will reach out to patients and families via MyChart who are eligible for the program.  After you complete our electronic survey, you can reach Care Plus by calling (415) 215-5802.

How can Care Plus’ services help me?

Through Care Plus you can be connected to an array of services and resources such as:

Social Work Consultations

Brief telephone meetings with a social worker who can direct you to resources and aid you in solving practical problems.  Practical problems can include issues you may be experiencing related to work, housing, food, transportation, finances, insurance, or getting help at home. Can include supportive counseling.

Social Work consultations generally take place on Thursdays between 1pm and 3pm.  Call Care Plus at (415) 215-5802 to learn more and determine eligibility.

Resiliency Classes for Patients

Resiliency for People Living with Serious Illness is a live-online support program of six weekly 60-minute classes.

In each class, you will learn evidence-based skills to care for yourself and sustain yourself throughout your illness.  Click here to view the flyer and learn more.

Sustainable Caregiving for Care Partners of People Living with Serious Illness is a live-online program of eight weekly 90-minute classes.

Caregiver Toolkit

As a caregiver, you are also part of your loved one’s health care team. The Caregiver Toolkit is designed to give you a better sense of co​ntrol and to prepare you for what to expect.  The toolkit includes 7 sections and a list of helpful resources. Advance Care Planning Resources

What is Advance Care Planning?

Advance Care Planning supports adults at any age or stage of health in understanding and sharing their personal values, life goals, and preferences regarding current and future medical care.  The goal is to help ensure that people receive medical care that is consistent with their values, goals, and preferences during serious and chronic illness.  It also involves talking about these goals with family, friends and medical providers.

Why is Advance Care Planning Important?

Advance care planning is considered a standard part of good healthcare and ideally should be done by all adults and before a health crisis.  Similar to having a safety back-up plan, advance care planning can help you get the medical care that is right for you at any stage in your health and at the end of life.​​​​​​

We offer two simple ways of learning more about advance care planning and an easy way to complete an advance directive:

Prepare for Your Care: An Easy-to-use Advance Care Planning Tool

Prepare for Your Care (PREPARE) is an online resource that helps people begin the important process of planning for medical care.

Using video stories, PREPARE helps people explore their personal wishes and learn how to discuss them with family, friends, and medical providers.

Join Redwing Keyssar, RN, for a free, in-depth Advance Care Planning Workshop to help you make medical decisions for yourself and your family.

This two-session workshop (offered monthly) enables you to:

  • Create a personalized plan to honor your wishes
  • Have your questions answered
  • Complete your new or revised Advance Directive for Healthcare

Symptom Management

If you have significant physical or emotional symptoms we can help, either by alerting your primary care team or by offering a referral to Palliative Care or Symptom Management.

What is Palliative Care & Symptom Management?

Palliative care and Symptom Management are types of specialized medical care aimed at improving quality of life in the face of a serious illness. Examples of serious illnesses include, but are not limited to, cancer, dementia and other neurological illnesses, heart failure, chronic lung disease, liver disease, and kidney failure. The palliative care and symptom management teams assist with physical and emotional symptoms, help people make decisions about their medical care, and provide support to patients and family members throughout the course of an illness.

Care Coordination

The Care Plus care navigator can also support you as you navigate the healthcare system, by helping to find answers to your questions and getting you connected with appropriate resources or services.  You are always welcome to reach out to us at (415) 215-5802 and we will also reach out to you periodically, by phone, to inquire about whether you have any needs that we could help you with.
